The Pharmacy First service allows community pharmacists to assess and manage a range of common conditions without the need for a GP appointment. For eligible patients with sore throat symptoms, pharmacists can provide a clinical assessment, offer advice and, where appropriate, supply treatment.


The aim is to help patients access care more quickly while ensuring that those who require further medical attention are referred appropriately.

Can a Pharmacist Assess a Sore Throat?

Yes. Pharmacists are trained healthcare professionals who can assess sore throat symptoms and determine the most appropriate course of action. Not every sore throat requires treatment, and many improve naturally without antibiotics — but some infections may benefit from further intervention, which is why a proper assessment is important.


During a Pharmacy First consultation, the pharmacist will review your symptoms and consider factors such as symptom severity, duration and any associated features that may suggest a bacterial infection.

Who Is Eligible for Treatment Through Pharmacy First?

The Pharmacy First sore throat service is available for patients aged 5 years and over who are experiencing symptoms of an acute sore throat. During your consultation, the pharmacist will ask questions about your symptoms, medical history and any existing health conditions to ensure Pharmacy First is appropriate for you — helping to ensure that patients receive safe and effective care based on their individual circumstances.

How Pharmacists Assess Sore Throat Symptoms

One of the key parts of the consultation is determining how likely it is that symptoms are being caused by a bacterial infection. To do this, pharmacists use a recognised clinical assessment tool called the FeverPAIN score — which supports appropriate antibiotic use and helps ensure treatment decisions are based on clinical evidence rather than symptoms alone.

The FeverPAIN score assesses

  • Fever during the previous 24 hours
  • Pus or white coating on the tonsils
  • Attendance within three days of symptom onset
  • Severely inflamed tonsils
  • Absence of cough or cold symptoms

The score helps identify patients who may be more likely to benefit from antibiotic treatment.

When Might Treatment Be Offered?

If your assessment suggests that a bacterial infection is likely, the pharmacist will consider whether antibiotic treatment is appropriate — based on your symptoms, your FeverPAIN score and whether you meet the eligibility criteria for the Pharmacy First service.

Patients more likely to be considered for antibiotics are those with:

Fever
Severely inflamed tonsils
White or pus-like coating on the tonsils
Symptoms that started recently and developed quickly
An absence of cough or cold symptoms

Most sore throats are caused by viral infections, which do not respond to antibiotics and usually improve on their own within a few days. Where antibiotics are unlikely to provide benefit, the pharmacist may recommend alternative treatments to help manage symptoms.

  • Pain relief medicines such as paracetamol or ibuprofen
  • Medicated throat sprays
  • Throat lozenges
  • Hydration and self-care advice
  • Other over-the-counter remedies to help relieve discomfort

The pharmacist will explain the reasoning behind their recommendation and advise on the most appropriate option based on your symptoms and clinical assessment.

Symptoms are unusually severe
You have difficulty breathing
You are unable to swallow fluids
Symptoms suggest a more serious infection

Symptoms do not fit the usual pattern of an uncomplicated sore throat
